There is nothing precise or accurate about a fitting without this step. □ Without this portion completed during the fitting, the provider is essentially “eye-balling” your prescription. Some Costco hearing centers are expected to perform at least a portion of this task, but I know for certain that this isn’t done in most cases as I have many patients with Costco hearing aids that I have to “fix” as this hadn’t been done. This involves placing microphones in the ear with the hearing aids to program them as precisely as possible according to your prescription. I do not fit a single patient with hearing aids without performing this testing and programming. This is 100% the gold standard in audiologic care. I would contact your Costco provider and ask them to perform “real ear measurements” using “real-ear-to-coupler-differences” (aka RECD). $1300 is still $1300 and my suggestions will help maximize the value you get from the hearing aids now and in the future. Here’s what I would do (as a perfectionistic, practicing audiologist with knowledge about audiologic best practices) to make sure you are getting as much value as you can from your purchase. In fact, sometimes, going through a hospital system or private practice can be more cost effective in the long run due to some of the restrictions imposed upon Costco’s hearing aid policies and the limitations placed on the providers.īottom line, you already have hearing aids through Costco and that is 100% fine. I can’t argue with that, but I also understand and hope to educate patients that their best interest may not always be the cheapest option, for a variety of reasons that I won’t go fully into here. Costco offers the bare minimum for the bare minimum price. □ What most don’t realize is that you definitely get what you pay for in the audiology world - typically what you are paying for is not only a product, but even more importantly, you are paying for the professional’s expertise and services. But if this persists only when the hearing aids are in, that would spark some concern for inappropriate amplification. A little fussiness or crying when the hearing aids are first put on is normal. Worst case scenario, if an audiologist somehow does make a mistake and fit an infant with inappropriately set hearing aids, it is typically pretty easy to tell if hearing aids are a little on the loud side as otherwise normally developing babies will be come excessively fussy if the gain is set too high. But generally speaking, an audiologist with any doubts related to test results would not fit hearing aids until test results have been confirmed. There are some cases where audiologists may question the ABR results for a variety of reasons (such as in OPs case). Each of these factors means that it would be quite difficult to fit an infant with hearing aids that could potentially cause auditory damage. There are also other objective tests used to cross check the results of the ABR. This is generally done by an ear nose and throat physician after care has been established, particularly for cases where the cause of hearing loss is unknown. Also, infants (as well as any child under 18) need medical clearance for hearing aids before they can be fit. ABRs are generally repeated to ensure reliability within the first few months of an initial diagnosis.
